RFP 6: Q&A
Important note to all RFP 6 bidders:
Please note to include any and all PUC Fees or other fees/taxes in your bid numbers. The FCC WILL subsidy 85% of all fees/taxes provided they are included in the winning bid. If they are not included in the bids, the FCC will NOT provide subsidy towards them which means that how those fees and taxes will be paid will need to be a discussion between the vendor and the site. In addition the FCC is stating that the site should NOT be responsible for paying these taxes/fees as they were not included in the bid.
Question:
Can you verify items numbered 1-22 on pages 13-18 on RPF #4 are identical to items numbered 1-22 on pages 13-18 on RPF #6?

That is correct, they are identical.
Question:
We have all of our documentation together and are getting ready to submit. I need to double check: Is there a specific form that needs to be filled out or a special way of submitting this proposal? Everything we have is on standard Company forms.
Answer:
All submission requirements are detailed on pages 5-11 on both RFP’s. I would strongly suggest reading this in its entirety. As to your specific question:
Each RFP Bid should have (see RFP 4/6 pages: 10,11):
· An original (marked “Original”) and 3 more paper copies.
· A CD containing the proposal in its entirety in a single PDF document.
· In addition: “The envelope/package containing the response must be clearly marked “Response to RFP for Oregon Health Network.” At least one original Proposal shall be signed in blue ink by an authorized representative of the Proposer. Alterations or erasures shall be initialed in ink by the person signing the Proposal. Proposals may not be submitted by telephone or fax.”
· All proposals must be delivered to:

Question:
Can you just verify the specific due dates for each RFP? I had a question from one of my partner companies about the date.

RFP 4 is due on 3/24/2010 by 4pm at the OHN Offices. RFP 6 is due on 3/23/2010 by 4pm at the OHN Offices. Just for completeness (I’m sure you already know this) the OHN Offices are located at: 2 Centerpointe Dr. Suite 570 Lake Oswego, OR 97035
Please note the following change to OHN RFP#6:
Central Oregon Community College has requested that we remove them from the RFP round and are requesting no bids at this time.
FCC Seeks Public Comment for Changes to National Broadband Plan
The Federal Communications Commission (FCC) announced it is making changes to the National Broadband Plan, which is designed to assist thousands of health providers at hospitals and clinics in the U.S. who lack adequate broadband access and capability. The announcement, made July 15, begins a 30-day open window for public comment. Updates to the FCC’s National Broadband Plan include the development of a permanent $400 million fund to further broadband infrastructure and build out of health information capabilities in rural areas. This financial support is an additional funding opportunity, outside of the current Rural Health Care Pilot Program (RHCPP) of which Oregon Health Network (OHN) manages for the state, and the standard Rural Health Care Program (RHC).
