For the latest information on who’s live on our network – this page is designed to help participants and regional leadership to understand who is on OHN – and therefore how best that network community could use OHN to improve the patient experience/outcomes, improve population health and reduce costs (Triple Aim). 

• Which OHN participants are you already working with, and how can OHN better support your current and future services with them?
  
• Who on OHN would you LIKE to work with, and now can, due to your one OHN connection (highway) to the entire OHN participant community?

At this point in time where all health care and education providers, private, industry and government alike, need to provide better coordinated services, to more patients, with less money  and resources– OHN is one of our State’s best core health IT resources in helping providers and communities to work more strategically and collaboratively to do more and better with less.

The following member sites are either actively being monitored by our Network Operations Center (NOC) or have received their formal Funding Commitment Letter (FCL) by the FCC and are awaiting final installation and NOC turn-up. Sites marked with an (*) are live on the network and being monitored by our NOC. In total, 241 sites are actively participating with OHN, with 141 sites actively being monitored by the NOC. The map above illustrates the participating sites, with green indicating a live site.

If you are participating in the FCC RHCPP and don't see your organization's name listed, don't worry. Many of the final RHCPP participants are at various stages in the process and waiting on finalizing a contract or awaiting the final funding commitment letter (FCL) from the FCC (USAC). Until we receive the FCL, we are not posting your site/organization's name on the site. If you need more information on the status of your site in any of 5-core RHCPP steps/phases, please contact us.
